On-Line Transportation Services Request Form

A Transportation Request Form must be completed by the parent for each student who will ride the bus, or when any changes need to be made.  The drop off address on this form must match the address on file with the campus before this form will be processed.  The parent/guardian is responsible for notifying the campus and the Transportation Department at 361-364-6834 fo any address or phone number changes that are made during the school year.
